INTRODUCTION:
According to Annex 15 of the EU-GMP Guide, Risk Assessment is a systematic process used to identify, analyze, evaluate, control and review potential risks associated with equipment, utilities, systems and manufacturing processes that may affect product quality, patient safety, data integrity and regulatory compliance.
Failure Mode and Effects Analysis (FMEA) is one of the most widely accepted Quality Risk Management (QRM) tools described in ICH Q9 (Quality Risk Management). It provides a structured methodology for identifying possible equipment failures, evaluating their impact on product quality and determining appropriate mitigation measures before qualification and routine operation.
The Blister Packing Machine is one of the most critical pieces of equipment used in the Oral Solid Dosage (OSD) pharmaceutical manufacturing process. It is designed to automatically pack tablets or capsules into PVC/Aluminium (PVC–Alu) or Aluminium/Aluminium (Alu–Alu) blister packs by forming pockets, feeding products, sealing lidding foil, embossing or printing batch details, perforating, cutting and discharging finished blister packs.
The quality of blister-packed products directly depends upon the consistent performance of the Blister Packing Machine. Any malfunction may result in:
- Incorrect product feeding
- Missing tablets or capsules
- Double feeding
- Improper pocket formation
- Improper sealing
- Foil wrinkles
- Leakage in blister packs
- Incorrect batch coding
- Poor print quality
- Improper perforation
- Incorrect cutting
- Product contamination

Therefore, a comprehensive Failure Mode and Effects Analysis (FMEA) is essential to identify all GMP-critical risks associated with the design, installation, operation, cleaning, maintenance and qualification of the Blister Packing Machine.
This Risk Assessment forms the basis for Design Qualification (DQ), Installation Qualification (IQ), Operational Qualification (OQ), and Performance Qualification (PQ), ensuring that all GMP requirements are adequately addressed prior to routine pharmaceutical manufacturing.
OBJECTIVE OF RISK ASSESSMENT
The objective of this Risk Assessment is to identify and evaluate all potential failure modes associated with the Blister Packing Machine that may adversely affect:
- Product Quality
- Patient Safety
- Equipment Performance
- Process Consistency
- Regulatory Compliance
- Operator Safety
- Environmental Protection
The specific objectives are:
- To identify all GMP-critical equipment components.
- To evaluate potential failure modes during installation, operation and cleaning.
- To determine the likelihood and impact of each identified failure.
- To establish suitable mitigation measures.
- To ensure compliance with WHO GMP, EU GMP, PIC/S GMP and ICH Q9.
- To verify that adequate engineering controls are incorporated into the equipment design.
- To ensure that all critical GMP requirements are verified during IQ, OQ and PQ.
- To minimize product contamination and cross-contamination.
- To ensure operator safety during routine production.
- To establish documented evidence supporting equipment qualification.
The outcome of this assessment will provide documented evidence that all foreseeable risks have been adequately identified and controlled before commercial pharmaceutical packaging.
