Failure Mode and Effects Analysis (FMEA) is one of the most widely accepted Quality Risk Management (QRM) tools described in ICH Q9 (Quality Risk Management). It provides a structured methodology for identifying possible equipment failures, evaluating their impact on product quality and determining appropriate mitigation measures before qualification and routine operation.
The Auto Capsule Filling Machine is one of the most critical pieces of equipment used in the Oral Solid Dosage (OSD) manufacturing process. It is designed to automatically separate empty hard gelatin or HPMC capsules, accurately fill them with powders, granules, pellets or similar pharmaceutical formulations, securely lock the capsule body and cap, and discharge finished capsules for subsequent polishing, inspection and packing.
The quality of filled capsules directly depends upon the consistent performance of the Auto Capsule Filling Machine. Any malfunction may result in:

OBJECTIVE OF RISK ASSESSMENT
The objective of this Risk Assessment is to identify and evaluate all potential failure modes associated with the Auto Capsule Filling Machine that may adversely affect:
