INTRODUCTION:

    According to the definition given in Annex 15 of the EU-GMP Guide, Risk Assessment is a systematic method used to identify, evaluate and control risks associated with equipment, utilities and manufacturing processes that may affect product quality, patient safety and regulatory compliance. Risk Assessment forms an integral part of the qualification and validation lifecycle and supports implementation of Quality Risk Management (QRM) principles described in ICH Q9.

    This Failure Mode Effect Analysis (FMEA) has been prepared for the Rapid Mixer Granulator (RMG) installed in the pharmaceutical manufacturing facility. The purpose of this assessment is to identify potential failure modes that may occur during the design, installation, operation, cleaning, maintenance and qualification of the equipment and to evaluate their possible impact on product quality, operator safety and Good Manufacturing Practice (GMP) compliance.

    The Rapid Mixer Granulator is one of the most critical pieces of equipment used for wet granulation. Since it directly influences granule characteristics such as particle size distribution, bulk density, moisture content and blend uniformity, any failure associated with the equipment may significantly affect the quality of the finished pharmaceutical product.

    This document identifies potential risks associated with the equipment and recommends suitable mitigation measures to reduce these risks to an acceptable level before qualification and routine commercial manufacturing.

    OBJECTIVE OF RISK ASSESSMENT

      The objective of this Risk Assessment is to ensure that all critical GMP, Engineering, Environmental Health & Safety (EHS), Operational and Quality requirements related to the Rapid Mixer Granulator are adequately identified and evaluated before qualification and routine operation.

      The assessment has been performed to:

      • Identify potential failure modes associated with the Rapid Mixer Granulator.
      • Evaluate risks that may directly or indirectly affect product quality.
      • Identify critical process parameters requiring qualification.
      • Verify that the equipment design complies with current GMP requirements.
      • Assess risks associated with cleaning, maintenance and calibration.
      • Evaluate operator safety risks during equipment operation.
      • Recommend suitable mitigation measures to minimize identified risks.
      • Define verification activities during IQ, OQ and PQ.
      • Ensure that residual risks remain within acceptable limits.
      • Provide documented evidence supporting Design Qualification and Validation activities.
