INTRODUCTION:

    Quality Risk Management is a systematic approach for identifying, evaluating, controlling, communicating and reviewing risks associated with pharmaceutical manufacturing equipment and processes.

    Failure Mode and Effects Analysis (FMEA) is a structured risk-management tool used to identify potential failure modes, their possible causes and effects, and the controls required to prevent or detect such failures.

    The Multi Mill is an important processing equipment used in pharmaceutical manufacturing for size reduction, milling, de-lumping and controlled particle-size reduction of granules and other suitable pharmaceutical materials.

    Failure or improper operation of the Multi Mill may result in:

    • Improper particle-size distribution.
    • Excessive or insufficient milling.
    • Generation of excessive fines.
    • Coarse particles remaining in the material.
    • Overheating of material.
    • Contamination.
    • Cross-contamination.
    • Metal contamination.
    • Foreign-particle contamination.
    • Product mix-up.
    • Incorrect screen selection.
    • Incorrect impeller/knife configuration.
    • Equipment damage.
    • Unplanned production stoppage.
    • Operator safety hazards.
    • GMP and product-quality risks.

    Therefore, a detailed FMEA is required to identify potential risks associated with the Multi Mill and establish appropriate preventive and detective controls.

    OBJECTIVE OF RISK ASSESSMENT

    The objectives of this FMEA are:

    1. To identify potential failure modes associated with the Multi Mill.
    2. To identify the causes and effects of each failure mode.
    3. To assess the potential impact on product quality and GMP compliance.
    4. To identify critical equipment components and operating parameters.
    5. To evaluate risks associated with milling operation.
    6. To establish appropriate preventive controls.
    7. To establish appropriate detection controls.
    8. To minimize contamination and cross-contamination risks.
    9. To minimize the risk of incorrect particle-size distribution.
    10. To control risks associated with screen, impeller/knife and drive-system failures.
    11. To establish appropriate cleaning and inspection controls.
    12. To establish preventive-maintenance requirements.
    13. To ensure operation by trained personnel.
    14. To support qualification and validation activities.
    15. To establish acceptable residual risk.
